You must cancel at least 24 hours prior to your scheduled visit or you will be charged a $50 no-show fee for office visits and $100 for procedures.

We allow a fifteen (15) minute grace period. If you arrive more than 15 minutes late for your appointment, it will be at the physician’s discretion whether you will still be seen or will have to reschedule.

Please bring your insurance card, completed forms, and your copay (if applicable). Please bring any previous medical records pertinent to your visit. For all refills, please call your pharmacy, they will fax us a refill request. Please allow 48 hours for your request to be filled. If we must see you before we allow a refill our office will contact you to set up an appointment.
